Field approval for 8.50 x 6 tires on a PA-12

Does anyone have paperwork for a field approval to put 8.50 x 6 tires on a pa-12 using Cleveland brakes ? I'm trying to get field approval using the STC that uses Hayes with a supplement for using Clevelands so any paperwork would be a great help . Thanks ,Erik
